To delete your existing listing: 

1. Log into the MyASHM Prescriber Portal 

2. Select the square ‘Practice Information’. 

3. You will see the practice locations you are associated with 

4. Select the arrow on the right-hand side of the listing 

5. Select ‘delete’ from the drop-down menu
