Queensland Positive People (QPP) are hiring Case Managers – HIV Support Services – Far North Queensland.

Queensland Positive People (QPP) are seeking a passionately motivated case managers to provide client-centred support to people living with HIV who are experiencing barriers to HIV treatment and care. QPP are offering two (2) full time permanent roles, with option to negotiate part time (0.6FTE minimum). These roles are located in either Cairns or Townsville, with outreach throughout the Far North and Central Queensland regions. We are recruiting two Case Managers in either a Peer Treatment Facilitator (PTF) or Treatment Support Facilitator (TSF) role.

About the Roles

Case Manager – Treatment Support Facilitator (TSF)
The TSF role focuses on client empowerment and the delivery of complex case management support for people experiencing challenges such as mental health or AOD issues, financial or housing stress, and unemployment. The role involves building tailored support plans with clients to address the social determinants of health that impact HIV treatment uptake, adherence, and retention in care.

Case Manager – Peer Treatment Facilitator (PTF)
The PTF role must be filled by a person living with HIV who is comfortable sharing their lived experience with clients and community. The role combines peer navigation and case management, providing holistic support, peer education, and practical assistance to navigate the health system. PTFs work collaboratively with clients to build tailored support to address the social determinants of health that impact HIV treatment uptake, adherence, and retention in care.

Position Details

  • Full-time (38 hours) weekdays or part time (minimum 0.6 FTE). Some out of hours and weekend work is required.
  • Fixed-term contract to 30 June 2027 (continuation of employment depends on continued program funding).
  • Work includes outreach (both in person and by phone), regular team connections (in person and online), and the promotion of work/life balance with flexible working arrangements.
  • SACS Award Level 5.1 ($51.00 per hour) + super + salary packaging to increase your take home wage.

Essential Requirements

  • Tertiary qualification in social work/community development and/or social/human services with prior experience in case management.
  • Hold a current QLD Working with Children Check (Blue Card).
  • Provide a National Police Check of 6 months currency or evidence that a check has been applied for.
  • Provide a valid Driver’s license with no restrictions.
  • Willingness to work outside normal business hours as required.
  • Ability to travel within Queensland and inter-state as required.
  • *To be eligible for the Peer Treatment Facilitator role, you must be a person living with HIV.
