Queensland Positive People (QPP) are looking for a compassionate, capable Aged Care Navigator to join the LIFE+ team, supporting older people living with HIV as well as LGBTQIA+ community members to understand, access, and stay connected to the care and supports they need as they age. This position is based in Meanjin (Brisbane), with some travel required.

- Part time (3 days per week)
- Fixed-term contract to 30th June 2027 (continuation of employment depends on continued program funding).

As an Aged Care Navigator, you will provide specialist, intensive support to people who need extra help engaging with My Aged Care and related community services. You’ll work closely with clients over time, building strong relationships, undertaking case management, and advocating to ensure services truly meet individual needs.
Working in a small team of dedicated staff and volunteers committed to helping people from all works of life live and age well, you will bring a strong knowledge of the latest research and best practice on ageing, HIV, LGBTQIA+ and Sistergirl and Brotherboy health. You will also have an excellent understanding of current reforms and ongoing implementations of change within the aged care sector.
You’ll collaborate with Care Finder partners across the consortium, contribute to system integration across health and aged care, and help create safer, more inclusive pathways for people ageing with HIV and for LGBTQIA+ communities.

Essential Requirements
- Evidence of a tertiary qualification in human services or relevant experience in health, aged care or social services.
- A valid Driver’s license with no restrictions.
- A National Police Check of six months currency (or international check if lived overseas in the last 12 months) or evidence that a National Police Check has been applied for.
- A current Queensland Blue Card or equivalent interstate Working with Children Check.
- A current Disability Screening Check (Yellow Card) or evidence that a check has been applied for.
- Demonstrated understanding and sensitivity of historical and contemporary HIV and support issues relating to people living with HIV as they age.
- Demonstrated understanding of the Aged Care Sector and support services, including an understanding of navigation/access issues, enablers, and barriers specific to the care finder target group.
- Demonstrated experience in assertive outreach and case management, with the ability to engage people respectfully and build trust over time.
- Demonstrated experience in maintaining exceptionally high standards of client confidentiality and privacy, recognising their critical importance in building safe and trusting relationships.
- Communicate clearly and ethically, using reflective, collaborative and transparent decision-making in your practice.
- Strong time management, organisational and reporting skills, with an ability to work independently, and are comfortable managing your workload in a remote work environment while actively engaging in supervision.
